If you are eligible, what are you required to bring?

Acceptable Identification for Status Card Issuance For Adults (16 + )

Acceptable ID for Children (under 15)

To get your new Status Card, you MUST bring in your valid government issued Identification — See the above documents for acceptable identification.
DO NOT come in unless you have them. A card will not be issued without valid ID —– NO EXCEPTIONS.

ONLY You yourself can come in and apply. 15 years & under must have their parent(s)/guardian/Foster with them, and the parent(s)/guardian/foster must provide valid ID (See above). The guardian/foster needs also to provide custody papers. Note: NEWBORNS, 2nd ID can be the hospital Bracelet The ABOVE Rules ensures that ONLY the rightful owner is issued and using the card. Helping to make sure that our benefits are protected.

What type of cards does Lyackson First Nation issue?

Currently Lyackson First Nation can only issue the laminated style of status cards as the Secure Certificate of Indian Status (SCIS) cards are only available in certain areas.
